Please take a moment to complete our new patient paperwork before your initial visit.  If your computer cannot read or print the documents, please come 15 minutes prior to your appointment, or feel free to come by our office to pick up hard copies of the paperwork to fill out.  This will help keep your appiontment time to a minimum.
